What gets discussed

Your goals, disability support needs, current supports, the supports proposed for your plan, your budgets, how funding will be managed, plan length, any funding periods, which providers you want recorded, and what information they may access with your consent.

Think about your everyday support needs

Personal care, communication, mobility, meals, household activities, community participation, employment or education, relationships, health related disability supports, and building independence. Think about what you can do alone, what you need help with, and how often.

Describe your hard days, not your good ones

Understating how you are managing is the single most common reason people end up with less support than they need. The plan reflects what you describe.

Gather evidence

  • Reports from treating health professionals
  • Allied health and functional assessments
  • Information about your current services
  • Provider reports
  • Quotes for proposed supports
  • Information about changes in your health or circumstances
  • Records showing what has and has not worked
  • Information about risks if support is not provided

Questions worth asking

  • Which supports are included, and why was something not included?
  • Which funding is flexible and which is stated?
  • How long is the plan, and do funding periods apply?
  • How will each funding component be managed?
  • What evidence is still required?
  • How can I ask for a change?
  • What can I do if I disagree with a decision?

Accessibility

Tell the NDIA beforehand if you need an interpreter, Auslan, Easy Read information, a communication aid, extra time, a support person, an accessible location, or a phone or online meeting. You have the right to receive information in a way you understand.

After the meeting

Write down anything you agreed to provide, send evidence promptly, keep copies, read the approved plan carefully, check the budgets and management arrangements, and ask for written reasons if a decision is unclear.
