Managing and reviewing a plan

Changing Your Plan

You can ask the NDIA to change your plan at any time if your circumstances, goals or needs have changed.

1

You do not need to wait

You can ask before your scheduled reassessment date.

2

Small and large changes differ

A variation for smaller changes, a reassessment for broader ones.

3

Evidence may be required

The NDIA may ask for information showing what changed and why.

Plain language guide

The detail, broken into useful sections.

Read the sections that matter to you. The content has not been shortened or changed, only organised so it is easier to scan.

01

When a plan might need to change

Your support needs or functional capacity have changed, your living arrangements or informal supports have changed, you are starting or leaving work or education, you need different equipment, your current supports are no longer appropriate, you need urgent or temporary support, you want to change how funding is managed, information in your plan is wrong, or your goals have changed.

A change in your life does not always require a change to your funding. The NDIA may first consider whether existing flexible funding can meet the need.

02

Plan variation

Changes part of your current plan — correcting an error, updating goals, changing your reassessment date, changing funding management, adding or changing part of your statement of supports, providing short term support after a sudden change, or adding crisis funding where appropriate. It does not necessarily replace your whole plan.

03

Plan reassessment

Used for broader changes, and may result in a new plan with different supports, funding amounts, categories, management arrangements, funding periods and reassessment date. The outcome can also be a variation, or no change.

04

How to ask

Speak with your my NDIS contact, call 1800 800 110, submit an enquiry through the NDIS service hub, visit an office or partner, or submit the relevant change of details or change of situation form.

Explain what changed, when, whether it is temporary or ongoing, how it affects daily life, why your current plan no longer meets your needs, which supports need to change, and what evidence you are providing.

If the change is urgent

Say so clearly — health or safety at immediate risk, essential informal support suddenly stopping, leaving hospital, losing access to essential equipment, or a living arrangement breaking down. Call 000 if someone is in immediate danger.
